For Journeypersons requesting an Electrician Trainee Card (ET Card), please read the following instructions, and complete the approval form by clicking the link at the bottom of this page

Please note, the $25.00 payment to the DIR – Electrician Certification Fund MUST BE IN CHECK OR MONEY ORDER FORM! The $175.00 payment to the SMJATC may be in check, money order, or credit/debit form. CASH IS NOT ACCEPTED!

INSTRUCTIONS:

1. Either type (fillable PDF) or write in the required information

a) Include your name, FULL mailing address, and your email (required for your online learning account)
b) Please choose a password of at least 6 characters, containing both letters and numbers (required for your online learning account)

2. Scan or take a photograph of the form and attach it to an email addressed to BOTH of the following addresses:

michael@smjatc617.org   AND  valerie@smjatc617.org

3. Await confirmation of approval of the request by a staff member of the SMJATC

4. Once approved by the SMJATC, you will be notified via email when you can come to the office to complete the ET Card DIR application form.

Normal ET card paperwork hours are from 8:00am to 4:30pm Mon – Fri. Please do not come to the office unless you have been pre-approved by the SMJATC staff.

Please note that the Dept of Industrial Relations ET card will NOT BE PROVIDED to you at the time of paperwork completion. The SMJATC will submit the forms and payment on your behalf and the DIR will mail or email your card to you if you are approved. This process generally takes 4 – 6 weeks.

After completing the Dept of Industrial Relations ET card application form at the SMJATC, the SMJATC will provide you with an interim ET card handout for use while the DIR processes your application
